After a medical emergency, patients commonly move from one setting of care to another such as from the hospital to a rehabilitation or long-term care center or home – this is referred to as “transitions of care (TOC)”. TOC is an important area that pharmacists play a key role in to ensure patients are on the right medication treatment and reduce medication errors.
Join this session with Dr. Clark & Dr. Nguyen as they walk through a patient’s care from the hospital to home and back to the primary care setting and highlight the roles of both an acute care and ambulatory care pharmacist, as part of the medical team.
